چکیده

Radiol Bras. 2017 Jan/Fev;50(1):VII–VIII Chronic liver inflammation, which is responsible for various stages of parenchymal fibrosis and, especially end-stage liver disease (cirrhosis), is the main cause of hepatocarcinogenesis and therefore of hepatocellular carcinoma (HCC), the most common primary malignant neoplasm of the liver. The causes of chronic liver inflammation include not only viral hepatitis but also chronic alcoholism, nonalcoholic steatohepatitis, and several other, less common, conditions. It is extraordinary to realize that with the technological advancement of the devices—mainly those of computed tomography (CT) and, especially, magnetic resonance imaging (MRI), through quantitative techniques and the various types of sequences available—radiologists have powerful noninvasive tools that function as “magnifying glasses” for the indirect study and precise evaluation of the cytoarchitecture of the organs of the body, with special attention to the liver. As detailed and demonstrated by Ramalho et al., in their article published in this issue of Radiologia Brasileira, MRI findings indirectly reproduce histopathological and hemodynamic changes, including altered hepatic morphology, fibrosis, and nodules, the spectrum of which ranges from regenerative nodules to dysplastic nodules of varying grades (from low to high) and, ultimately, HCC. The authors described the various advantages of MRI in relation to CT, including the fact that the former does not involve the use of ionizing radiation, has higher tissue resolution (which can facilitate the identification of intralesional fat components), and allows the use of extracellular and liver-specific contrast agents, as well as an excellent evaluation of the biliary tree by cholangiography. They also demonstrated the superiority of MRI over CT in the diagnosis of small HCCs, as well as in the detection of dysplastic nodules and diffuse HCC. Despite the importance of the abovementioned characteristics in the MRI sequences, the key tool for the diagnosis of HCC— and therefore for the evaluation of nodules in chronic liver diseases— is undoubtedly the study of the hemodynamic behavior against the

Evaluation of serum AFP (α-fetoprotein) level in HBsAg carrier patients for diagnosis of hepatocellular carcinoma

Background: Hepatocellular carcinoma (HCC) is common all over the world as well as Iran. The incidence of HCC is higher in hepatitis B carriers and it is highly recommended to periodically screen these patients by serum alpha-fetoprotein (AFP) and liver ultrasound (US) every 6 months. We explore the diagnostic accuracy and the performance of (AFP) in cases of hepatitis B carriers as a scree...

Fine-Needle Aspiration Biopsy of Hepatocellular Carcinoma and Related Hepatocellular Nodular Lesions in Cirrhosis: Controversies, Challenges, and Expectations

The role of hepatic fine-needle aspiration (FNA) biopsy has evolved. Advances in imaging modalities have obviated the need for tissue confirmation in most hepatocellular carcinomas (HCCs). There is risk of needle-tract seeding. Increasingly, small nodules are being detected on ultrasound surveillance of high-risk patients. Diagnostic challenges associated with cirrhosis include distinction of b...

Accuracy Rate of Clinical Diagnosis of Disc Displacement without Reduction in Temporomandibular Joint Using MRI

Introduction: Internal derangement (ID) of TMJ is the most common type of temporomandibular disorders after muscle disorders and includes all disorders related to incoordination and dislocation of disc and condyle. Anterior disc displacement without reduction or closed lock will happen if the disc or condyle displaces severely or if the disc traps in the space in front of the condyle accompanie...

Magnetic resonance imaging of the cirrhotic liver: diagnosis of hepatocellular carcinoma and evaluation of response to treatment - Part 1

Magnetic resonance imaging (MRI) is the modern gold standard for the noninvasive evaluation of the cirrhotic liver. The combination of arterial phase hyperenhancement and delayed wash-out allows a definitive diagnosis of hepatocellular carcinoma (HCC) in patients with liver cirrhosis or chronic liver disease, without the requirement for confirmatory biopsy. That pattern is highly specific and h...

O-19: A Comparison of Pelvic Magnetic Resonance Imaging, Trans-Vaginal and Trans-Rectal Sonography with Laparoscopic Findings in The Diagnosis of Deep Infiltrating Endometriosis

Background The performance of TVS, TRS and MRI for the diagnosis of DIE have been separately reported in earlier studies. However, the three methods have not been concurrently compared in terms of their overall performance and classified results as per different anatomical locations of DIE lesions in a large study population. This study was an attempt to compare pelvic magnetic resonance imagin...
